formatting requirements outlined in NIH Notice

This new format is required for applications submitted for due dates on
or after May 25, 2015.
I tried to mimic the example documents provided on the [SF 424 (R&R)
Forms and Applications
page](http://grants.nih.gov/grants/funding/424/index.htm#format) as
closely as possible. I have used this class for my own grant
submissions; however I offer no guarantee of conformity to NIH
requirements.

- Functions deprecated in 2014 removed (package authors have
been informed of this change)
- Case changing code revised to give more useful behaviour when
working with UTF-8 input with pdfTeX and with LICR accents
across all engines. The latter are now recognised 'directly'
by the code and so need no special handling. For UTF-8 input,
the code path for pdfTeX will now case change any code point
that can be typeset in the T1 font encoding.
- Retain whitespace in lines when processing with l3build. This
will require adjustment to existing .tlg files, and is
necessary to give better coverage of line normalisation
between LuaTeX and other engines.
- Fix behaviour of l3galley and addvspace/addvpenalty
