Update of the minitoc package to version 43. First version in .ins/.dtx format. Added new commands. Added new language files (.mld). Fixed several bugs. Improved the hints option. Improved documentation.view m*.dtx. Added scripts to make build the package and its documentation. Added INSTALL and a chapter about installation.
