Changes since v0.5:

* Added the Ingredient command for single-argument ingredients.

* Fixed a couple of vertical alignment problems.

* Labelling of recipes for ef. Added different index and contents options, and corrected a bug that sometimes gave the wrong page number.

* X (times) macro.

* If recipe's #4 is empty, don't leave a blank line.

* The step number width can be changed but the recipe number width varies.
