The version 2.00 of Mathmode comes with the sorce files.

with Herbert's agreement, i have installed this version of mathmode in a directory of its own. users may browse the directory via

http://www.tex.ac.uk/tex-archive/info/math/voss/mathmode/

and the document itself is at

http://www.tex.ac.uk/tex-archive/info/math/voss/mathmode/Mathmode.pdf

the files were installed a while ago, and will probably be available via other ctan sites by now.
