Piet van Oostrum submitted the new version 3.1 of his fancyhdr package. It contains a small patch to solve a problem with verbatiminput and includegraphics.
fancyhdr is a package for extensive control of page headers and footers in LaTeX. It is installed on CTAN in
macros/latex/contrib/fancyhdr
