coverage and support for stacking accents and ties above and below

- Match the math comma and semicolon with text mode
- Made the QED symbol to absorb current text color

- Make sure that the bookmark of index is at the root level
- Improve default math font setting, notably for the configuration of `mathcal` font
- Add a new option for selecting print mode, along with some related commands
of TINYINT(1) conditions ; this would be useful if one wishes to have different
page layout parameters while printing
- Improve the hyperref setup in fast mode
- minor fixes and internal improvements
The (currently out-of-date) documentations (also for the packages `lebhart` and `beaulivre`) are removed to prevent misleading.
